The HLC is both, a community hub focusing on supporting the local wards of Southcourt and Walton Court in Aylesbury and they are also committed with their outreach work on the wards of Oakridge & Castlefield in High Wycombe and Vale in Chesham.
The aim is to tackle health and wellbeing inequalities in areas of Buckinghamshire which rank amongst the 30% most disadvantaged in the England.
